Health classes explained
The four main rate tiers, what each one represents, and what factors determine your placement.
Carriers organize pricing by rate class. While names shift between companies, the structure stays the same: Preferred Plus (Super Preferred or Preferred Best), Preferred, Standard Plus, and Standard, then higher tiers for special circumstances. Tobacco users get a separate, pricier ladder.
What underwriters look at
Height, weight, blood pressure, cholesterol, heart disease or cancer before age 60 in your family, driving history, medications, work and activities, and any tobacco or nicotine use. The top tier wants near-perfect results across the board. Preferred is more flexible. Standard Plus and Standard accept managed conditions like controlled high blood pressure.
